Program FA
- Member will Apply online
- Member brings in income documentation.
- Staff cannot award Program FA. You can only inform them of what they qualify for. It is not guaranteed that they will be approved. Use this chart to determine what they qualify for.

Staff completes Documentation Review & Executive Director Approval Mach Form
- Complete all required fields
- Start Date- Enter a 6 month period ususally starting with today's date.
- Program Purchasing Group: Select whatever they are applying for.
- Applicant Qualifies For: Select what they qualify for. Certain branches do not award 70% for programming.- Check with your branch Director.
- The appropriate Director or branch approver will ultimately approve/deny the Program FA.
Membership FA
- No application required. Instead, a conversation takes place. Information regarding the Membership Audit* should be part of this conversation.
- Use this chart to see what they qualify for. Staff may award 30% or 50% membership FA.
- If they qualify for 70%, offer them 50% so that they can get approved on the spot. Only Executive Directors or Membership Directors may award 70%. Certain branches do not award 70% for membership.-Check with your branch director.
- To award the 30% FA or 50% FA, staff complete this form: Purchasing Group Assistance
- This will immediately add the 30% or 50% FA Purchasing Group to Personify.
- The person can join in YMCA membership after this Purchasing Group form has been completed by staff.
- Requests for 70% FA: Complete the Financial Assistance Documentation Review & Executive Director Approval
Notes
- Membership FA award lasts two years. Program FA lasts 6 months.
- Membership Joining Fees are no longer waived with the membership FA award. Instead, Joining fees are discounted the same percentage off that was awarded to them, 30%, 50%, or with Director approval, 70%.
- *All awardees are subject to a Membership Audit which can happen at any time during the two-year award period. When selected to participate, members will have 30 days to bring in their financial documentation in for review.
FA Status Request

Membership FA:
- Membership FA status’ requests should be sent to ED/Membership Director.

Youth Sports/Swim Lessons/Other Branch Programs FA:
- Any FA status requests for swim lessons, youth basketball, karate, and other youth programs that take place at the branch should be sent to the ED/Healthy Living Director.

Youth Development FA: Camp Campbell Camps, Summer Day Camps, Break Camps, Afterschool Care:
- Send status request to financialassistance@ymcasv.org. BRANCH DOES NOT APPROVE CAMP OR AFTERSCHOOL FINANCIAL ASSISTANCE.
